My wife’s 1st vaccination certificate had maiden name + married name
2nd maiden name only
Flu vaccine both names!

Weird isn't it !
My pass sanitaire is in my maiden name (I haven't kept the first certificate so can't remember the name there) but my flu jab is in my married name. Both have the my carte vitale reference number ....
If I'd known before coming to France what a faff a surname could make, I'd have kept my maiden name !

My PS is in both names, my Vaccine Passport is in my maiden name.
The tax office uses my maiden name so does EDF.
C'est la vie.

The irony is when I spoke to CPAM they said maiden name is the rule and it can’t be changed( despite having both names on 1st vac cert and married name on Carte Vitale) Our MT was unable to change it and wrote her married name on the 2nd vac certificate along with his stamp.
